There are six secret levels in Dungeon Keeper, 5 can be accessed by finding "reveal hidden realm" specials, the 6th can only be accessed in this fashion:
1: Set your computer clock to any calendar day that is a full moon.
2: Set it to a couple minutes before midnight.
3: Run the game with the proper date and time.
Before the game starts, the game will display a small part of an extremely long Dungeon Keeper epic poem (for the entirety, examine the .txt files in Dungeon Keeper for a file that contains it). Then, go to Start New Game.
There will be TWO level flags now, the normal one for Eversmile, and a flag with a full moon on it. Click this second flag to start in the super secret level.
In this level, your task is to out-mine the other keepers while under constant attack by Dwarves. Lightning is your only defence! If you don't have enough gold by the time limit, you lose.
Once you beat this level, you advance to the normal 1st level and continue as normal. Enjoy!
P.S. I've also seen the map with a faded-out version of the Super Secret Level flag, I'm only guessing but I think this may be because you are very close to a full moon. I know that this works with Dungeon Keeper Gold edition, which is what I have. I have not tried it with the original.
